SCAN Health Plan has been serving their members since 1977, when several frustrated seniors decided to form a company that met their health care necessities. The result was the Senior Care Action Network, today called the SCAN Health Plan. The main goal of SCAN has remained the same since the beginning, to develop creative ways of allowing seniors to administer their own health care and to manage their lifestyles. SCAN has been able to provide necessary health services to provide over 50,000 seniors with the ability to remain outside of skilled nursing facilities.

 

SCAN currently provides coverage to more than 110,000 people in several California counties as well as Maricopa County,Arizona. They are currently the provider of the fourth biggest nonprofit Medicare Advantage health care program in the US.

 

SCAN has their headquarters in Long Beach. They currently employ about 800 people and serve California residents of Kern, Los Angeles, Orange, Riverside, San Bernardino, San Diego, and Ventura. They also serve Arizona’s Maricopa County. SCAN Health Plan extended its services to northern California, including Santa Clara, San Francisco, and Contra Costa counties throughout 2009. By acquiring Medcore Health plan, SCAN was able to expand coverage and is proud to be serving over 2,000 additional individuals in San Joaquin County.

 

SCAN has been recognized for their exceptional service. U.S. News & World Report ranked SCAN #131 in Medicare Health Plans for their Classic, Options, and SNP offerings. They were also ranked #133 in Medicare Health Plans for their Legacy plan.

SCAN offers My Choice plans in Los Angeles, Riverside, San Bernardino, and Orange Counties. This is an HMO-POS plan that offers a $3,000 initial drug coverage, and an in-network out of pocket maximum of $1,050. There is no out of network limit. Doctor visits are a $10 copay in network, and $40 out. Specialist visits are $20 in network, and $40 out.

 

SCAN also offers a special plan for those on both Medicare and Medi-Cal. This plan offers the enrollees very low drug costs, no monthly plan premium, no copay for doctor visits, skilled nursing, inpatient hospital, emergency, outpatient, ambulance, vision and hearing, dental, and more. There are special eligibility requirements to enroll in this plan.

 

In Contra Costa, San Francisco, and Santa Clara the Classic and Options plans are offered. Both plans include hearing and vision with copays and restrictions, both plans also have drug benefits with initial coverage up to $2,900. The Classic plan also has monthly premiums ranging from $39 to $59 depending on the area. All doctor visits have a $10 copay, while specialists have a $15 copay. The Options plan has a monthly premium that ranges from $29 to $49 depending on the area. There is no copayment for doctor or specialist visits.

 

In Kern and Ventura only the Classic plan is offered. The monthly plan premium is $20 in Kern, and there is no premium in Ventura. To see the doctor there is a $10 copayment for both the doctor and specialist in Ventura. In Kern the copayment is $5 for the doctor and $10 for the specialist. The initial drug coverage limit is $2,900 in Kern, and $3,000 in Ventura.

 

In Los Angeles Classic, Options, My Choice, and Medicare/Medi-Cal are offered. The Classic plan offers no monthly premium, $5 doctor and $10 specialist copayments and a $4,000 initial drug coverage limit. The Options plan offers no monthly premium, no doctor or specialist copayments and an initial drug coverage limit of $2,900.

 

In Orange County Classic, Options, and My Choice are offered. The Classic plan offers $3,000 initial drug coverage limit, no monthly premiums and a $5 doctor and $10 specialist copayment. The Options plan offers $2,900 initial drug coverage limit, no monthly premium and no copayments for doctor or specialist visits.

 

In Riverside and San Bernardino Classic, Options, My Choice, and Medicare/Medi-Cal are offered. The Classic plan offers no monthly premium, a $3,000 initial drug coverage, and $10 doctor and specialists copayments. The Options plan offers no monthly premium, no copayments for doctor or specialist visits, and a $2,900 initial drug coverage limit.

 

In San Diego Classic and Signature are offered. The Classic offers a $2,900 drug coverage limit. It also has a $25 monthly plan premium as well as $10 doctor copays and $25 specialist copays. The Signature offers a $3,000 drug coverage limit. It also has a $69 monthly plan premium as well as a $15 doctor copay and a $35 Specialist copay.

 

In San Joaquin Gold, Silver, and Select are offered. The Gold plan offers no monthly plan premium, a $3,000 out of pocket limit and $10 doctor copays and $30 specialist copays. The Silver plan is very similar to the Gold plan, but with no drug coverage. The Select plan offers very low drug costs, no monthly premium, and no copay for doctor visits.
