In Missouri there are three plans available. The PPO Gold plan, the PPO Silver plan, and the HMO plan.
The PPO Gold plan has an extensive network, offers preventive screenings, an annual physical, worldwide emergency and urgent care. This plan also offers out of network benefits, as well as benefits though the prescription coverage gap, and is offered as a plan with or without part d coverage. This plan also does not require referrals.
The PPO Silver plan also has the extensive network, preventive screenings, annual physical, worldwide emergency and urgent care. This plan also offers out of network benefits, but does not offer benefits though the prescription coverage gap. There is a $0 premium with the part d coverage. This plan also does not require referrals. HMO plan also has an extensive network, preventive screenings, annual physical, worldwide emergency and urgent care. This plan does not offer out of network benefits, and also does not offer benefits though the prescription coverage gap. It does have a $500 annual cost for hospitalization, and offers plans with or without part d coverage. This plan also does not require referrals.
In Arkansas there are two plans offered. Plan 1 has no monthly premium and a maximum out of pocket expense of $4,800. The copayment to visit a primary care physician office is $15 in network and 20% out of network. An annual routine physical is $0 in network and 20% out of network, as are preventive services, x-ray, lab services, and home health care. There is also drug coverage with this plan.
Plan 2 has an $83 monthly premium with the maximum out of pocket expense of $4,800. The copayment for a primary care physician office visit is $5 in network, and $15 out of network. Annual routine physicals are $0 in network and $5 out of network. For preventive services, they are $0 in network, and anywhere from $0-$15 out of network. X-ray services are $0 in network and 20% co-payments out of network, as are lab services. There is also drug coverage with this plan.
