Clarian Health was formed in 1997 as a collaboration of three downtown Indianapolis hospitals, Methodist Hospital, Indiana University Hospital, and Riley Hospital for Children. All three hospitals are preeminent, state-of-the-art hospitals with long and distinguished histories. They joined together to provide better benefits to the patients and the entire community through shared resources, research, staff and administrative procedures. It was named on the “Best Hospitals in America” list released by USNews & World Report for five consecutive years.
Clarion Health is a private, nonprofit organization with ties to more than 20 hospitals and health centers in their network, including close ties to the Indiana University School of Medicine. More than half of the IU School of Medicine's 970 physicians are at one of Clarion’s Hospitals to practice, supervise, and teach residents. In addition, the research conducted through the IU School of Medicine leads to the breakthroughs that make a difference in the lives of Clarian's patients.
Taking excellence in patient care, education and research to the next level, Clarian Health opened Clarian West Medical Center in 2004, followed by Clarian North Medical Center in 2005, and Lafayette's Clarian Arnett Hospital in the fall of 2008. There have been many "firsts" in medicine, including Indiana's first kidney, liver, heart and bone marrow transplants that were performed at Clarian Health hospitals.
Clarian Health Plans offer Medicare Advantage plans for Medicare beneficiaries. These plans are only available in certain Indiana counties including: Blackford, Boone, Carroll, Delaware, Hamilton, Hendricks, Howard, Lawrence, Marion, Monroe, Morgan, Tippecanoe, Tipton, and Vigo Counties.
Offered in these fourteen counties are four different Medicare Advantage plans. These plans are Clarian Medicare Value, Clarian Medicare Select, Clarian Medicare Select Plus, and Clarian Medicare Choice.
Clarian Medicare Value is an HMO Medicare Advantage plan that offers members the benefits of coverage with no monthly premium for the plan. You are still responsible for the Medicare Part B premium however. There are also no deductibles and there is a $3,400 out-of-pocket limit on this plan. To visit your primary care physician there is a $35 copay, and a $55 copay for specialist visits. This plan does not offer Medicare Part D prescription drug coverage. Medicare Part B drugs are covered at a 20% coinsurance.
Clarian Medicare Select is an HMO Medicare Advantage plan that offers members coverage at a low monthly premium. The monthly premium is $37.20 plus the monthly Medicare Part B premium. There are also no deductibles in this plan, and there is a $3,400 out-of-pocket limit. To visit your primary care physician there is a co-pay of $20 per visit, and for specialists it is $40 per visit. This plan does not offer Medicare Part D prescription drug coverage. Medicare Part B drugs are covered at a 20% coinsurance.
Clarian Medicare Select Plus is an HMO Medicare Advantage that offers slightly higher costs, but covers more than some of the other plans. There is a $66.90 monthly premium in addition to the monthly Medicare Part B premium. There are no deductibles on this plan and there is a $3,400 out-of-pocket limit. To visit your primary care physician there is a $20 co-pay, and a specialist visit is $40. Medicare Part B drug coverage is offered at 20% co-insurance. Medicare Part D drugs under this plan have a $0 deductible. The initial coverage limit is $2,830, after reaching this you are responsible for all costs until you reach $4,550. You can reach your initial coverage limit with the 31-day supply prices of $5 for generic/tier 1drugs, $35 for preferred/tier 2 drugs, $70 for non-preferred/tier 3drugs, and a 30% coinsurance for specialty/tier 4 drugs. There are also set prices for 90-day supplies.
Clarian Medicare Choice is an HMOPOS Medicare Advantage plan that is only available in Marion, Boone, Hamilton, Hendricks, and Morgan counties. There is a $92.90 monthly premium in addition to the monthly Medicare Part B premium. There are no deductibles on this plan and there is a $3,400 out-of-pocket limit including POS. To visit your primary care physician there is a $10 co-pay, and a specialist visit is $35. Medicare Part B drug coverage is offered at 20% coinsurance. Medicare Part D drugs under this plan have a $0 deductible. The initial coverage limit is $2,830, after reaching this you are responsible for all costs until you reach $4,550. You can reach your initial coverage limit with the 31-day supply prices of $5 for generic/tier 1 drugs, $35 for preferred/tier 2 drugs, $70 for non-preferred/tier 3drugs, and a 30% coinsurance for specialty/tier 4 drugs.
