Bravo Health was founded in 1996 under the name Elder Health. Their name changed in 2007 to Bravo Health in order to more accurately represent the active nature of today's seniors. Bravo health is committed to providing Medicare beneficiaries with high quality, but also cost effective medical costs and care. They are headquartered in Baltimore, Maryland and currently serve over 300,000 members with their network of hospitals and 20,000 physicians. Their Medicare Advantage plans are offered in Delaware, Maryland, New Jersey, Pennsylvania, Texas, and Washington, DC. Bravo Health also offers a Part D prescription drug plan in 43 states.

 

What types of plans do they offer?

Bravo Health offers a varierty of options to fit the needs of their members. They offer several types of Medicare Advantage plans such as HMO (Health Maintenance Organization), PPO (Preferred Provider Organization), PFFS (Private Fee for Service), and POS (Point of Service), as well as Medicare Part D prescription plans.

 

Bravo Achieve is a Special Needs Medicare Advantage Plan HMO designed for those who meet specific requirements. For example, those with diabetes may be eligible for this plan. This plan has no monthly premium with low copayments for most services. This plan has no monthly premium with low copayments for most services. Diabetic supplies and related drugs have no copay required. With this plan you also have the option of enrolling in the optional Encore Package, which provides additional dental and hearing aid benefits.

 

The Bravo Choice Medicare Advantage Plan is an HMO plan that is great for those looking for lower premiums without losing benefits. This plan offers low physician copays as well as low cost-sharing for any inpatient and outpatient services. Also included in this plan are the Medicare Part D prescription drug plan, dental benefits, and membership to a gym.

 

Bravo Classic Medicare Advantage also has no plan premium while still offering a variety of benefits. There are set co-pays for most services, so there are no surprises and you can save money. Also included is the Medicare Part D prescription plan. With Bravo Classic you also have the choice of adding the Encore Package providing dental and hearing aid coverage.

 

Bravo Classic Plus Medicare Advantage Plan is an HMO and POS plan. There is no plan premium and set co-payments for most health care services. Also included in this plan is a Point-of-Service feature allowing you to visit physicians that are outside of Bravo’s network, doing so may possibly cost extra however. With this plan you can also elect to add Bravo’s optional Encore Package, which offers you added hearing and dental service coverage.

 

Bravo Freedom Medicare Advantage Plan is a PPO (Preferred Provider Organization) plan. This plan allows to you see doctors both in and out of the network, giving you a wide variety of choices. Also included in this plan is eye coverage, including glasses, contacts, and routine eye exams and hearing aid coverage. Medicare Part D prescription is also covered.

 

Bravo Gold is a HMO Medicare Advantage plan that does not include Medicare Part D prescription coverage. The plan has no plan premium and offers dental coverage and chiropractic services.

 

Bravo Liberty II Rx provides the same benefits as Original Medicare. This plan also provides a Medicare Part D Prescription Drug plan with no deductible and an $8.50 Generic drug copayment.

 

Bravo Premier Medicare Advantage offers the ability to have a lot of benefits at a reasonable premium. Included in this plan is the Medicare Part D prescription coverage, dental coverage and low set copays for doctor visits, outpatient services, as well as inpatient hospital stays. You also have the option of enrolling in Encore Package which offers additional dental coverage and hearing aid coverage.

 

Bravo Premier Plus Medicare Advantage Plan covers your Medicare benefits for a low monthly premium with low to no cost sharing. Included in this plan is the Medicare Part D prescription coverage, dental and hearing aid coverage, as well as low co-pays for doctor and specialist visits, outpatient services and inpatient hospital stays. This plan also offers members a POS (Point of Service) option that allows them to see a doctor outside of the network.

 

Bravo Select is a Special Needs Medicare Advantage Plan for those that have Medicare and receives medical assistance from the state. It has no plan premium and no cost-sharing for all benefits. This plan also includes the Medicare Part D coverage, provides transportation to and from doctor appointments, dental coverage, hearing aid coverage, and gym membership.

 

Bravo Silver is a package designed for people with limited income and qualify for government health insurance subsidies. It offers all Medicare benefits for no monthly plan premium, and low to no cost sharing. It also offers dental and vision coverage, membership at a fitness gym, as well as transportation for those who have no way to travel to and from the doctor’s office themselves.

 

Bravo Traditions Medicare Advantage Plan is for those who are nursing home residents or who need a home health care equivalent to that of a nursing home resident. It includes the Medicare Part D coverage and can provide you with a ride to and from your appointment with your physician.
