The Universal Health Care Group offers health care in Arizona, Florida, Georgia, Louisiana, Maryland, Mississipppi, Nevada, Pennsylvania, South Carolina, Texas, and Utah.  There are three main medicare plans that Universal offers which are "Any..Any..Any..PFFS Plan," Medicare Masterpiece HMO and Medicare Masterpiece PPO.  Medicare Masterpiece HMO and PPO are only offered in Florida and Texas however.  The premiums and co-pays varies depending on where the beneficiary lives.  Those who are eligible for these plans are entitled to Medicare Part A and are enrolled in Medicare Part B.

 

Any..Any..Any..PFFS

 

This is a Medicare Advantage Private-Fee-For-Service Plan where a beneficiary has access to any Medicare providers that accept the terms of payment .  Within the Private-Fee-For-Service Plan there are three options: Medical Advantage only, Gold Medical Advantage Part D, and Platinum Medicare Advantage Part D.  MA does not include pharmacy coverage.

 

Medicare Masterpiece HMO

 

This HMO policy provides the same benefits included in traditional Medicare. Within the Health Maintenance Organization, there is Medicare Masterpiece HMO, Medicare Masterpiece HMO plan MA only, Medicare Masterpiece HMO Plan Premier, Medicare Masterpiece HMO Special Needs Plan, Medicare Masterpiece HMO Special Needs Plan-Diabetes, Medicare Masterpiece HMO Special Needs Plan-Dementia, and Medicare Masterpiece HMOPOS Plan Plus which allows the beneficiary to use doctors and hospitals outside the plan's network at an additional cost.  The Medicare Advantage Only Plan does not include a prescription drug plan.

 

Medicare Masterpiece PPO

 

Within the Preferred Provider Organization there is Medicare Masterpiece PPO, Medicare Masterpiece PPO plan MA only, Medicare Masterpiece PPO Plan Premier, Medicare Masterpiece PPO Special Needs Plan, Medicare Masterpiece PPO Special Needs Plan-Diabetes and Medicare Masterpiece PPO Special Needs Plan-Dementia. The Medicare Advantage Only Plan does not include a pharmacy plan.
