Clear One Health Plans was founded in 1995. Previously known as Clear Choice, they changed their name in September of 2009 to Clear One Health Plans. They offer individual, family and group plans throughout Oregon and Montana. They also offer Medicare Advantage Plans in Oregon. They have over 145 employees and serve more than 40,000 members.

 

Clear One was the first managed care plan nationwide to be owned and operated by physicians and hospitals. They have continued their dedication to the development of community-based health insurance plans by using local providers and quality clinical evaluations as a basis for their decision-making.

 

Clear One offers several Medicare Advantage plans to the residents of Oregon.

 

The Value Plan (HMO) Medicare Advantage plan offers a low cost plan that still meets your needs. There is a $15 co-pay for doctor visits and a $25 co-pay for specialists. In network hospitals have a set fee of $200/day for the first 4 days of the stay, after day 4 there is no fee. Out of network doctor and hospital services are not covered under this plan.

 

The Traditional Plan (HMO-POS) Medicare Advantage plan offers a $10 co-pay for doctor visits and a $25 co-pay for specialists. For out of network care a percentage is charged rather than a set amount. In-network hospitals have a set fee of $125/day for the first 4 days of the stay, after day 4 there is no fee. Out of network hospitals are not covered under this plan.

 

The Explorer+RX Plan (PPO) Medicare Advantage plan includes a prescription drug plan. There is a set fee for both in and out of network doctors and specialists. This is $20 for in-network and $30 for out of network doctors. For specialists the fee is $30 for in and $40 for out of network. In-network hospital stays have a fee of $500 per stay. This is also the only plan where out of network hospitals are covered at a 15% cost.

 

The Value+RX Plan (HMO) Medicare Advantage plan also includes a prescription coverage plan. Other than the inclusion of the RX coverage, the plan coverage is exactly the same as the Value Plan.

 

The Traditional+RX Plan (HMO-POS) Medicare Advantage also includes drug coverage on top of all the same coverage and copays as the Traditional Plan.
