HumanaChoice PPO R5826-063 policy involves a $0 premium.  To visit a primary care provider, there is $10 copay and you must also notify the physician. To see a specialist, there is $30 copay and members receive one annual physical exam and eye exam.  There is $550 copayment for each stay in inpatient hospital care and $0-$95 copay per outpatient visit.  Pharmacy is not covered by this plan. 

 

HumanaChoice PPO R5826-003 has $72 premium per month. In order to visit a primary care doctor, you must provide the $15 copay and notify the doctor. When you visit a specialist, there is $35 copay and members receive one annual physical exam.  You are responsible for $225 copayment for the first 7 days for impatient hospital care and $0-$250 copay per outpatient visit.  Pharmacy is included, see plan for details.
