Humana Choice R5826-078 Regional PPO has a monthly premium of $38. For primary care visits there is $15 copay when the primary care physician is notified. For specialist visits there is $40 copay and members receive one annual physical exam. There is $225 copay per days 1-7 for inpatient hospital care and $0-250 copay or per outpatient visit.
