HMO Humana Gold Plus H1036-011A had a monthly premium of $0. There is a $0 co-pay for primary care office visits and a $30 co-pay for specialty office visits. Hospital inpatients have a $195 co-pay per day for the first eight days of their hospital stay. Hospital outpatients have $50-$195 co pay per visit, depending on the cost of the visit. Pharmacy, such as Prescription Drug Coverage, is included with this Plan.
