The Humana Gold Plus HMO plan has a monthly premium of $0.  For primary care visits there is $20 co-pay when the primary care physician is notified.  For specialist visits there is $40 co-pay and members receive one annual physical exam.  There is $275 co-pay for the first 6 days for inpatient hospital care and $40-$200 co-pay per outpatient visit.

 

The Humana HumanaChoice Regional PPO plan has a monthly premium of $88.  For primary care visits there is $15 co-pay when the primary care physician is notified.  For specialist visits there is $35 co-pay and members receive one annual physical exam.  There is $150 co-pay for the first 7 days for inpatient hospital care and $100-$150 co-pay per outpatient visit.

 

The Aetna HMO Medicare Premier Plan has a monthly premium of $0.  For primary care visits there is $20 co-pay when the primary care physician is notified.  For specialist visits there is $40 co-pay and members receive one annual physical exam and one annual eye exam.  There is $195 co-pay for the first 7 days for inpatient hospital care and $200 co-pay per outpatient visit.

Humana Medicare began marketing health insurance products in 1984.  The monthly premiums for Humana’s plans range from $0-$147. 

 

The difference between HMO and PPO plans in Medicare is that PPO, or Preferred Provider Organization plans offer beneficiaries the choice of their primary care physician and the choice of what health care provider they would like to go to at a higher copay rate.  However, Health Maintenance Organization or HMO plans tend to be more affordable.
