Horizon’s HMO Blue Value w/ RX Standard has a monthly premium of $62.  For primary care visits there is $15 copay when the primary care physician is notified.  For specialist visits there is $35 copay and members receive one annual physical exam at $15 copay.  There is $195 per days 1-8 for inpatient hospital care and $35-125 copay or per outpatient visit.
