For specialist visits there is $35 co-pay and members receive one annual physical exam. There is $150 co-pay per days 1-5 for inpatient hospital care and $0-$125 co-pay per outpatient visit. HumanaChoice R5826-001 Regional PPO has a monthly premium of $80. For primary care visits there is $15 co-pay when the primary care physician is notified. For specialist visits there is $40 co-pay and members receive one annual physical exam. There is $225 co-pay per days 1-7 for inpatient hospital care and $0-250 co-pay per outpatient visit. HumanaChoice R5826-065 Regional PPO has a monthly premium of $0. For primary care visits there is $10 co-pay when the primary care physician is notified. For specialist visits there is $30 co-pay and members receive one annual physical exam and one annual eye exam. There is $550 co-pay per stay for inpatient hospital care and $0-95 co-pay per outpatient visit. Pharmacy is not included within this plan.
