Humana Health Insurance offers five Medicare Plans to your area. They are HMO Humana Gold Plus H1036-054C, PPO HumanaChoice R5826-074, PPO HumanaChoice R5826-018, and PPO HumanaChoice R5826-005, and HMO Humana Gold Plus H1036-034A. An HMO is a Health Maintenance Organization and a PPO is a preferred provider organization. These plans can be added to your Medicare Advantage Plan (Medicare Part C).

 

PPO HumanaChoice R5826-074 has a monthly premium of $84. There is a $15 co-pay for primary care office visits and a $35 co-pay for specialty visits. Hospital inpatients have $225 co-pay per day for the first seven days of their visit. Hospital outpatients have $0-$150 co-pay per visit, depending on the cost of the visit. Pharmacy, such as prescription drug coverage, is included with this Plan.

 

Medicare Supplement Plan F has a monthly premium of $280.85. There is $0 co-pay for office visits, hospital in patients, and hospital outpatients. Pharmacy is not included with this Plan.
