Humana Gold Plus H1806-002 PPO has a monthly premium of $30.  For primary care visits there is $10 copay when the primary care physician is notified.  For specialist visits there is $30 copay and members receive one annual physical exam. There is $195 per days 1-8 for inpatient hospital care and $100-150 copay or per outpatient visit.
