HumanaChoice R5826-065 Regional PPO has a monthly premium of $0.  For primary care visits there is $10 co-pay when the primary care physician is notified.  For specialist visits there is $30 co-pay and members receive one annual physical exam and one annual eye exam.  There is $550 copay per stay for inpatient hospital care and $0-95 co-pay per outpatient visit.  Pharmacy is not included within this plan.

 

Mutual of Omaha’s Plan F has a monthly premium of $142.32 and is accepted by all doctors who accept Medicare.

 

Mutual of Omaha was established in 1909 and has earned high ratings in A.M. Best, Moody’s and Standard & Poor’s through its time.

 

Medicare Supplement Plan F coverage for Part B deductible and excess charges above Medicare-approved amounts and Plan C offers coverage for Part B deductible.
