These office visits are classified as either a primary care or specialized (such as an orthopedic doctor or radiologist). You will have a different copayment depending on the type of visit. Hospital Inpatient coves an overnight stay at the hospital (your plan may specify a minimum number of nights needed in the hospital for you to be covered). Hospital Outpatient covers services due to a hospital visit that was not and over-night visit. It may also cover medical services that did not require hospitalization. If you receive Pharmacy coverage from your Plan, this means that your will receive coverage on your prescription drug costs.
What are the costs associated with my new Plan?
For the Plan HumanaChoice H5826-053 (Regional PPO), there is no cost of a monthly premium. A primary care office visit will have a $10 co-pay. A specialty office visit will have a $30 co-pay. If you use the services of a hospital inpatient, you will have a $550 co-pay for your visit (there is no variation for the number of nights you stay). A Hospital Outpatient will have a $50-$90 co-pay for each visit. This plan does not offer Pharmacy coverage of prescription drug costs.
HumanaChoice R5826-072 (Regional PPO) has a monthly premium of $96. You will have a $15 co-pay for any primary care office visit. A specialty office visit will have a $35 co-pay. Hospital Inpatients will be charged a $225 per day co-pay for days 1-7 of their hospital stay. Days 8-90 have $0 co-pay per day. The co-pay for hospital outpatients ranges from $100-$150 depending on the cost of each visit. Finally, in this Plan, Pharmacy benefits will cover your necessary prescription drug costs.
