If you choose to use a doctor or hospital outside of the network, your coverage may not be applicable. Humana Health InsuranceHumana Health Insurance offers three Medicare Plans: PPO HumanaChoice R5826-074, PPO HumanaChoice R5826-018, and PPO HumanaChoice R5826-005. A PPO is a Preferred Provider Organization, which allows people to choose their primary care physician. These plans can be added to your Medicare Advantage Plan.

 

PPO HumanaChoice R5826-074 has a monthly premium of $84. There is a $15 co-pay for primary care office visits and a $35 co-pay for specialty office visits. Hospital inpatients have a $225 co-pay per day for the first seven days of their hospital stay. Hospital outpatients have a $0-$150 co-pay per visit, depending on the cost of the visit. Pharmacy, such as prescription drug coverage, is included with this Plan.

 

Medicare Supplement Plan F has a monthly premium of $186.06. There is a $0 co-pay for office visits (both primary care and specialty), hospital inpatients, and hospital outpatients. Pharmacy is not included with this Plan.
