Members also receive one annual physical exam and one annual eye exam. You must pay $185 copayment for days 1-8 of inpatient hospital care and $250 of copay per outpatient visit. Pharmacy is not included within this plan.

 

Aetna Medicare Standard Plan PPO involves $123 monthly premium. If you notify your primary care doctor, there is $20 copayment per visit. If you visit a specialist, there is $35 copay. Aetna members are entitled to one annual physical exam and one annual eye exam. For the first seven days of inpatient hospital care, there is $200 copayment and $225 of copay per outpatient visit. One more common Aetna Advantage plan is the Medicare Premier Plan PPO, which has a premium of $158 per month. If you want to make an appointment with your primary care doctor, you must notify the doctor in advance and also pay $15 copayment. In order to see a specialist, you must pay $25 copayment. Again, Aetna rembers receive one annual physical exam and one annual eye exam. For the first seven days of inpatient hospital care, there is a copayment of $125, and a copayment of $200 for every outpatient visit. Pharmacy is included, see plan for details.

Humana Medicare premiums range from $0 to $95 per month for both HMO and PPO plans. Medicare Supplement Plan F offers basic copay coverage in addition to Part A hospital deductible, skilled nursing facility copays, Part B deductible and emergency international coverage.
