3. Horizon: Horizon offers six different Medicare Plans to your area. These are POS Blue Access w/Rx Standard, HMO Blue Value w/Rx Standard, POS Blue Access, POS Blue Access w/Rx Enhanced, HMO Blue Value, and HMO Blue Value w/Rx Enhanced. An HMO Plans is a Health Maintenance Organization. A POS is a combination of an HMO and a PPO (preferred provider organization).
Horizon offers POS Blue Access w/Rx Standard for a monthly premium of $127.10. There is a $15 co-pay for primary care office visits. Specialty office visits will have a $30 co-pay. Hospital inpatients will have $75 co-pay per day for the first ten days of their stay. Hospital outpatients will have a $35-$125 co-pay per stay, depending on the cost of each visit.
