For specialist visits there is $30 copay and members receive one annual physical exam and one annual eye exam. Aetna’s HMO Medicare Value Plan has a monthly premium of $0. For primary care visits there is $20 copay when the primary care physician is notified. For specialist visits there is $35 copay and members receive one annual physical exam and one annual eye exam. Aetna’s PPO Medicare Premier Plan has a monthly premium of $100. For primary care visits there is $20 copay when the primary care physician is notified. For specialist visits there is $40 copay and members receive one annual physical exam and one annual eye exam.
