There is $550 copay per stay for impatient hospital care and $0-$50 copay per outpatient visit. Pharmacy is not included.

 

HumanaChoice Regional PPO R5826-001 has a monthly premium of $80. For primary care visits there is $15 copay when the primary care physician is notified. For specialist visits there is $40 copay and members receive one annual physical exam. There is $225 copay per days 1-7 for impatient hospital care and $0-$150 copay per outpatient visit.
