PPO HumanaChoice H4520-003 has a monthly premium of $104. There is a $15 co-pay for primary care office visits and a $40 co-pay for specialty office visits. Hospital inpatients have $225 co-pay per day for the first seven days of their stay. Hospital outpatients have $100-$150 co-pay per visit. Pharmacy, such as Prescription Drug Coverage, is covered with this plan.

Gerber Life Insurance offers Medicare Supplement Plans A, F, and G. Medicare Supplement Plan F offers coverage on excess fees of your Medicare Plan. This includes fees you may encounter at doctors visits, such as “out-of-pocket” fees. It also offers coverage for your Part B deductible. Foreign travel emergency insurance is on of the many benefits of this plan. Medicare Supplement Plan F is one of the most popular Medicare Plan.

 

Medicare Supplement Plan F has a monthly premium of $118.69. There is $0 co-pay for office visits, hospital inpatients, and hospital outpatients. Pharmacy is not included with this plan.
